چکیده

Materials and methods Eighteen vertebrae were harvested from the lumbar section of six porcine spines ranged from L1 to L3. All vertebrae were instrumented on both pedicles using pedicle screws. Quantitative CT scans were used before instrumentation in order to assess the bone density of each vertebra. Cyclic bending (toggling) load of ± 1mm displacement at 3Hz for 5000 cycles were assigned in two directions including craniocaudal (CC) and transverse (TR) toggling. Twelve instrumented pedicles were selected for CC toggling, twelve pedicles for TR toggling and twelve pedicles were not toggled (NT). All toggled and non-toggled screws were then pulled out at a displacement speed of 5mm/min in longitudinal direction. The peak pullout force and stiffness were computed from the load-displacement curves. Analyses of variance (ANOVA) were performed to investigate the effects of toggling methods and vertebral levels on the pullout force and stiffness.

Pars and pedicle fracture and screw loosening associated with cortical bone trajectory: a case series and proposed mechanism through a cadaveric study.

BACKGROUND CONTEXT Cortical bone trajectory (CBT) technique for pedicle screw placement in the lumbar spine has become more popular since its introduction in 2009. The distinct advantages of using the CBT technique involve increased screw purchase within the cortical bone and reduced surgical dissection. However, contrary to several favorable biomechanical results, there were anecdotal reports ...

Pullout Strength after Expandable Polymethylmethacrylate Transpedicular Screw Augmentation for Pedicle Screw Loosening

OBJECTIVE Pedicle screw fixation for spine arthrodesis is a useful procedure for the treatment of spinal disorders. However, instrument failure often occurs, and pedicle screw loosening is the initial step of a range of complications. The authors recently used a modified transpedicular polymethylmethacrylate (PMMA) screw augmentation technique to overcome pedicle screw loosening. Here, they rep...

Evaluation of implant loosening following segmental pedicle screw fixation in adolescent idiopathic scoliosis: a 2 year follow-up with low-dose CT

BACKGROUND The long term radiological status of screw fixation following scoliosis surgery with all pedicle screw construct is not previously studied. AIM To evaluate the incidence of loosening (implant failure) evaluated with low-dose CT two years following scoliosis surgery. STUDY DESIGN Retrospective study. METHODS 81 consecutive patients with adolescent idiopathic scoliosis (AIS), age...

Biomechanical study of rotational micromovement of the pedicle screw

BACKGROUND In regard to the fixation using a pedicle screw (PS) and rod system, the mechanism from the onset of the clear zone up to the development of loosening of the pedicle screw is not completely clarified. The purpose of this study is to determine the cause of the pedicle screw loosening by performing a biomechanical study with three-dimensional movie analysis. METHODS Ten PS fixation m...

Biomechanical and Histological Evaluation of an Allograft Anchor for Pedicle Screw Augmentation in an Ovine Model

INTRODUCTION Achieving solid implant fixation to osteoporotic bone presents a challenge to both spinal surgeons and hardware designers. Loss of surgical construct stability as a result of screw loosening is a well-known complication [1,2], particularly in patients with poor bone quality [3,4]. Thus, the development of novel strategies to address the issue of obtaining enhanced bone-to-screw pur...
